MetLife Statement of Responsibility Form
MetLife now requires a new form for ALL business as of 7/1/15. It is needed whether it comes from MetLife miniSUITE, MetLife SUITE, or standalone MetLife Dental. It is called the Statement of Responsibility and can be found here. Furthermore, the CoPower team will be adding this newly required form to all MetLife Summary of Benefit material checklists as well as each MetLife Employer application.
